I think one of the worst models developed by Reason is the Swiss Cheese. No wonder safety is so attracted to it as if life is predictable, mechanical, linear and controllable. Safety is so prone to these seductions and don’t realise the assumptions behind such a framework nor the philosophy it is anchored to. Similarly, the delusions of a hierarchy of controls. I don’t think Safety understands either of these as embedding a philosophy that is undeclared in its design.
